TITLE:  RESIDENTIAL SUPERVISOR/QIDP

PROGRAM:  RESIDENTIAL                          

SUMMARY OF POSITION: Oversee the day-to-day operations of the Agency’s OPWDD regulated residences for persons with intellectual developmental disabilities. This position reports to the Residential Director & Assistant Director.

Ensure adequate coverage for all shifts, including holidays are met.  Hire, train, coach and discipline staff.  Schedule and facilitate meetings, including staff meetings, clinical/management meetings, resident meetings, etc.  Support our individuals to ensure their rights, health, safety & personal outcomes are being met under guidance of the IDT Team, Providers and OPWDD guidelines. In addition, ensure appropriate and individualized treatment plan is developed, monitored and revised as needed

MINIMUM REQUIREMENTS:  BA Degree in the human services field. Two (2) years of experience in an OPWDD program working in a supervisory capacity. Strong coordination skills required.

TITLE:  RN SUPERVISOR

PROGRAM: RESIDENTIAL

SUMMARY OF POSITION: Oversee coordination and supervision of all medical, dental and other health related services for the adult population with intellectual & developmental disabilities. Deliver and ensure all individuals’ care by assessing and diagnosing residents’ condition, developing plans of care and evaluating responses. Provide supervision to RNs, LPNs and other clinical staff. Successful candidate must be available for off-hour emergencies.

MINIMUM REQUIREMENTS:  Bachelor’s degree required. NYS Registered Nurse License and 3 years of experience working in an OPWDD setting is preferred.

TITLE:  DAY SERVICES ASSISTANT

PROGRAM:  DAY SERVICES – 164TH

SUMMARY OF POSITION: Provide comprehensive care to adults with developmental disabilities in a day program. Work with groups of individuals, organize activities and teach independent living skills such as money management, safety, social and communication. Plan and implement arts and crafts projects, assist individuals in the use of community resources, i.e. parks, library and other recreational facilities. Transport individuals to volunteer sites and teach them work skills at those sites. Ensure the safety of individuals in all settings by following specified supports and safeguards as delineated on Staff Action Plan.

Day Program Shifts – 7:30 am – 3:30 pm or 8:30 am – 4:00 pm

MINIMUM REQUIREMENTS: High School Diploma/GED or some college, studying in the field of Human Services. A valid New York State driver’s license for three (3) years is required. Prior work experience in a human service field working with individuals with developmental disabilities preferred. The ability to write daily entries or notes about the client’s services is required.

TITLE:  COOK/HOUSEKEEPER

PROGRAM:  RESIDENTIAL

SUMMARY OF POSITION: Provide meals for 10 individuals in their homes.  Will be trained in preparing different textures for individuals with swallowing difficulties. Assist with keeping on track with household budget when shopping and maintaining a clean house.  Position located in Bellerose, Queens.  Work schedule 10:00 am to 6:00 pm with days off on Friday and Saturday.

MINIMUM REQUIREMENTS: High School/GED diploma and NYS Driver’s License for 3 years, required.

TITLE:  DIRECT SUPPORT PROFESSIONAL

PROGRAM:  RESIDENTIAL

 SUMMARY OF POSITION: Provide comprehensive care to adults with intellectual and developmental disabilities in a day program or in their homes. Work on ADL goals, arts and crafts projects, zoom activities and assist with personal hygiene needs. Perform various housekeeping tasks and train and assist individuals in the use of community resources, i.e., parks, library and other recreational facilities. Teach our individuals independent living skills and have fun while teaching them.

VARIOUS SHIFTS AVAILABLE FOR RESIDENTIAL PROGRAM: Evenings (3:00 pm to 11:00 pm) & (4:00 pm -12:00 am)

Early mornings (6:00 am-10:00 am)

Weekends shifts (8:00 am – 4:00 pm) & (4:00 pm-12:00 am).

Day Program hours:  Mon to Fri, 8:30 am -3:30 pm

MINIMUM REQUIREMENTS: High School Diploma/GED required.  A valid New York State driver’s license for 3 years is strongly preferred. Prior work experience in a human service field, working with individuals with developmental disabilities preferred. TITLE:  LPN (FT/PT)

PROGRAM:  RESIDENTIAL

SUMMARY OF POSITION:  The LPN is responsible for medical services for each person living in the residences.  Monitors and supervises all medical treatments, appointments, including medication, nutrition, lab procedures, etc.  Ensures that a comprehensive medical/nursing plan is in place for each individual.  Work schedule is three (3) days (7:00 am to 5:00 pm) 3 days and one (1) Saturday overnight (10:00 pm to 8:00 am).

MINIMUM REQUIREMENTS:  Licensed Practical Nurse certification required.  High School Diploma/GED required.

TITLE:  QUALITY INTELLECTUAL DISABILITY PROFESSIONAL

PROGRAM:  QUALITY ASSURANCE

SUMMARY OF POSITION: 

The Quality Intellectual Disability Professional works closely with employees, coaching and training, to maintain and improve excellent quality standards in the execution of their job responsibilities.

DUTIES:

Develop and maintain training programs to meet each program’s need, in compliance with state and federal regulations

  • Design and monitor computer tracking system for training and other agency wide compliance issues
  • Assist in program audits and follow-up with training issues identified through noted deficiencies
  • Assist in investigations and subsequent training as needed
  • Maintain a liaison with outside governing agencies by attending committee meetings, training and conferences as needed
  • Provide agency wide training as needed
  • Provide technical assistance to each program as needed
  • Perform all other duties as assigned by supervisor.

MINIMUM REQUIREMENTS:  BA/BS Degree required. Minimum one year of experience working in OPWDD programs required. Successful candidates should have good writing, communication and interpersonal skills. Must have training experience and knowledge of OPWDD rules and regulations.

TITLE:  DSP- PRE VOCATIONAL TRAINER

PROGRAM:  PRE-VOCATIONAL                                                                                               

SUMMARY OF POSITION: The DSP Pre-Vocational Trainer is assigned to individuals with developmental disabilities to work and train them on the required tasks to help prepare them for a job in the community in the future.  The DSP maintains daily records of attendance and productivity; drives individuals to and from sites; provides written and verbal reports on individual’s progress and performance; assume responsibility for quality control.

MINIMUM REQUIREMENTS: High School Diploma/GED and prior working experience with adults with intellectual developmental disabilities in a vocational setting required, as well as, a NYS Driver’s license for 3 years.

TITLE:  HUMAN RESOURCES RECRUITER

PROGRAM:  HUMAN RESOURCES

Hours: Mon -Fri, 8:30 am to 4:30 pm and as needed. Must work In Person.

SUMMARY OF POSITION: Responsible for carrying out recruitment, hiring and on-boarding processes to ensure the Agency maintains a skilled and diverse workforce to carry out its mission.

RESPONSIBILITIES INCLUDE: Develop and implement recruiting strategies for potential employees. Screen resumes, set up interview appointments and follow-up. Collaborate with the Agency’s programs and services to identify hiring needs.

MINIMUM REQUIREMENTS: High School/GED Diploma; college credit strongly preferred. Five (5) years of experience in recruiting for agencies in the health care field. Familiarity with Applicant Tracking Systems and resume databases. Proficiency with social media platforms. Excellent verbal and written communication skills.

TITLE:  HUMAN RESOURCES MANAGER

PROGRAM:  HUMAN RESOURCES

SUMMARY OF POSITION: The Human Resources Manager will lead and participate in the daily functions of the Human Resources (HR) department including recruiting, overseeing benefits administration, and disseminating and enforcing Agency policies and practices. In conjunction with the Director of Human Resources, oversee the scheduling, assignments, daily workflow of HR Department staff.

DUTIES & RESPONSIBILITIES: Under the supervision of the Director of Human Resources, organizes, coordinates, facilitates, and oversees HR staff in carrying out all HR Department functions
including but not limited to: Recruitment, Retention, Recording and maintenance of
Documentation, Onboarding, Benefits, Leaves of Absence, and Safety.

Partners with the Leadership team to understand and execute the organization’s
Human Resource strategy as it relates to current and future staffing needs, recruiting,
and retention. Handles employment-related inquiries from applicants, employees, and supervisors, referring complex and/or sensitive matters to the Director of Human Resources.  Provides support and guidance to HR staff and other Agency staff when questions and issues arise; may be required to administer and execute routine tasks. Manages the Recruitment process, which includes advertising, outreach, interviewing, and background checking of applicants; collaborates with Program, Directors and Supervisors to understand skills and competencies required for openings. Oversees and participates in recruitment and onboarding data tracking system. Maintains compliance with federal, state, and local employment laws and regulations and recommended best practices; reviews policies and practices to maintain compliance.  Maintains knowledge of trends, best practices, regulatory changes, and new technologies in human resources, and employment law.  Performs other duties as assigned.

REQUIRED SKILLS/ABILITIES:  Excellent verbal and written communication skills. Excellent interpersonal, negotiation, and conflict resolution skills.  Excellent organizational skills and attention to detail. Strong analytical and problem-solving skills.  Ability to prioritize tasks and to delegate them when appropriate. Ability to act with integrity, professionalism, and confidentiality.

Thorough knowledge of employment-related laws and regulations.  Proficient with Microsoft Office Suite or related software.  Proficiency with or the ability to learn the organization’s HRIS and recruitment and onboarding systems.

EDUCATION & EXPERIENCE:  Bachelor’s degree in Human Resources, Business Administration, or related field required.  A minimum of three years of Human Resource supervisory experience preferred.

TITLE:  SOCIAL WORKER

PROGRAM:  ARTICLE 16 CLINIC

SUMMARY OF POSITION: Under the supervision of the Article 16 Clinic Director, deliver planned intervention to persons with intellectual and developmental disabilities to resolve or ameliorate social problems encountered. The method of service delivery is through individual, and/or family counseling.

 DUTIES:

  • Conduct intake/screening to potential clients and assist in defining the presenting problem or treatment
  • Conduct psycho-social evaluations and arrange for additional clinical evaluations needed by the client
  • Provide ongoing individual and /or group counseling to at least 30 assigned cases
  • Develop Individual’s annual treatment plan based on a thorough needs assessment and review TX plan every 6 months
  • Update and maintain case records in accordance with Part 679 regulations
  • Provide information/referral services to persons with developmental disabilities and their families
  • Provide family counseling services to those individuals duly admitted to the clinic experiencing familial conflict and stressors.

MINIMUM REQUIREMENTS

To be successful in this role, you should have the following qualifications:

  • Master’s Degree in social work
  • One year work experience in OPWDD
  • Valid NYS Social Work License
  • Excellent communication and interpersonal skills

TITLE:  PSYCHOLOGIST

PROGRAM:  ARTICLE 16 CLINIC

Successful candidates must have experience in the administration of the following tests:

  • Stanford-Binet Intelligence Scales
  • Wechler Scales, Comprehensive Test of Nonverbal Intelligence
  • Vineland Adaptive Behavior Scales – Revision 3
  • Wide Range Achievement Test – Revision 5

You will:

  • receive training and ongoing supervision
  • use test instruments to conduct assessments
  • work with children and adults

SUMMARY OF POSITION: Render psychological services to persons diagnosed with Intellectual Developmental Disability (IDD) and their families. Psychological testing is a critical function in order to ascertain cognitive and adaptive ability of the individual. Evaluation reports are used to qualify for NYC, state, and federal programs and services. Psychologist will also render counseling service and utilize treatment methods of individual and family counseling to address and resolve problems of personal adjustment, interpersonal conflicts, social relations, and over all social functioning.

MINIMUM REQUIREMENTS: MA or MS Degree in Psychology and one year experience in psychological testing and counseling of people with developmental disabilities.

TITLE:  INTAKE AND CURRICULUM COORDINATOR

PROGRAM: BELLEROSE DAY PROGRAM       

SUMMARY OF POSITION: To facilitate DH intakes into program and develop a DH training curriculum.

DUTIES: The Intake and Curriculum Coordinator is responsible for intake and screening into program: Meet with applicants and provide a tour of program. Ensure admission paperwork is complete, i.e. LP, LOC and OPW authorization and approvals. Facilitate intake meetings and enter new admissions and discharges into Choices database, paperwork into Precision Care and check medical through E-paces. Attend monthly QCP Intake Committee meetings, aging out fairs and monthly DD Council meetings. Develop and create a DH program curriculum for small groups in program rooms and teach DSP’s how to implement and maintain an active curriculum and schedule throughout the day in program rooms. Facilitate meetings with Supervisor and DH Specialists to develop creative programming. As assigned by Director/Assistant Director, assist group rooms with daily activities. If needed, assist Supervisors in the completion of daily, monthly and semi-annual documentation.

MINIMUM REQUIREMENTS: Bachelor’s degree in Human Services Field such as psychology, social work, education, etc. from accredited University and experience in the field required.
