TITLE:  DAY SERVICES ASSISTANT

PROGRAM:  DAY SERVICES – 164TH

SUMMARY OF POSITION:

  • Provide comprehensive care to adults with developmental disabilities in a day program
  • Work with groups of individuals, organize activities and teach independent living skills such as money management, safety, social and communication
  • Plan and implement arts and crafts projects
  • Assist individuals in the use of community resources, i.e. parks, library and other recreational facilities.
  • Drive individuals to volunteer sites using Agency vehicles and teach them work skills at those sites
  • Ensure the safety of individuals in all settings by following specified supports and safeguards as delineated on Staff Action Plan.

Day Program Shifts – 7:30 am – 3:30 pm or 8:30 am – 4:00 pm

MINIMUM REQUIREMENTS:  

  • High School Diploma/GED or some college, studying in the field of Human Services.
  • A valid New York State driver’s license for three (3) years required for transport of individuals to volunteer sites.
  • Prior work experience in a human service field working with individuals with developmental disabilities preferred.
  • The ability to write daily entries or notes about the client’s services is required.

TITLE:  DIRECT SUPPORT PROFESSIONAL

PROGRAM:  RESIDENTIAL

SUMMARY OF POSITION: Provide comprehensive care to adults with intellectual and developmental disabilities in a day program or in their homes. Work on ADL goals, arts and crafts projects, zoom activities and assist with personal hygiene needs. Perform various housekeeping tasks, and train and assist individuals in the use of community resources, i.e., parks, library and other recreational facilities. Teach our individuals independent living skills and have fun while teaching them.

VARIOUS SHIFTS AVAILABLE FOR RESIDENTIAL PROGRAM:

Evenings (3:00 pm to 11:00 pm) & (4:00 pm -12:00 am)

Early mornings (6:00 am-10:00 am)

Weekends shifts (8:00 am – 4:00 pm) & (4:00 pm-12:00 am).

Day Program hours:  Mon to Fri, 8:30 am -3:30 pm

MINIMUM REQUIREMENTS:

  • High School Diploma/GED required
  • A valid New York State driver’s license for 3 years is strongly preferred
  • Prior work experience in a human service field, working with individuals with developmental disabilities preferred

TITLE:  LICENSED CLINICAL SOCIAL WORKER (LCSW) – In Person

PROGRAM:  ARTICLE 16 CLINIC

We are seeking a compassionate, skilled and dedicated Licensed Clinical Social Worker (LCSW) to join our Article 16 Clinic, where person-centered care is at the heart of everything we do.

POSITION SUMMARY: The Licensed Clinical Social Worker will conduct clinical evaluations and deliver structured therapeutic interventions to individuals with intellectual and developmental disabilities, as well as their families and significant others. Services are primarily provided through individual and family counseling, with the goal of addressing and alleviating emotional, social, and behavioral challenges in a supportive clinical setting.

KEY RESPONSIBILITIES:

  • Provide information and referral services to individuals with IDD and their families
  • Conduct intake assessments and screenings, helping to identify presenting concerns
  • Complete comprehensive psychosocial evaluations to determine OPWDD eligibility and recommend appropriate services
  • Deliver individual and/or family counseling services
  • Develop, implement, and regularly review individualized treatment plans
  • Maintain accurate and up-to-date case records in compliance with Part 679 regulations

REQUIRED QUALIFICATIONS:

  • Master’s Degree in Social Work (MSW) from an accredited institution
  • Current New York State licensure (LCSW) required
  • Minimum of one year of experience working in the IDD field
  • Strong communication, interpersonal, and organizational skills
  • Preferred: Bilingual (English/Spanish)

TITLENUTRITIONIST (RDN/CDN) – Article 16 Clinic

PROGRAM:  ARTICLE 16 CLINIC

SUMMARY OF POSITION: We are seeking a passionate and knowledgeable Nutritionist to join our team. In this vital role, you will provide comprehensive nutrition assessments, counseling, and ongoing support to individuals receiving clinic services. This role focuses on promoting health, preventing nutrition-related complications, and ensuring dietary needs are met in accordance with medical, behavioral, and developmental considerations.

SCHEDULE:  5-7 Hours per week.  In person

DUTIES:

  • Complete thorough nutrition assessments that integrate medical history and dietary intake information and develop individualized nutrition plans. Maintain accurate, timely progress notes, plans, and evaluation reports and complete documentation in compliance with the Article 16 Clinic regulations (NYS OPWDD), Medicaid/insurance billing requirements and Clinic policies and timelines
  • Promote healthy eating habits tailored to developmental abilities and cultural backgrounds.
  • Offer both individual and group nutrition counseling using a person-centered approach that promotes positive health behaviors and outcomes and adjust plans as needed.
  • Develop and deliver nutrition education programs, workshops, or materials for individuals, families, and staff.

MINIMUM REQUIREMENTS:

  • Bachelor’s or Master’s degree in Nutrition, Dietetics, or a related field
  • Credentialed as: Registered Dietician Nutritionist (RDN) OR Certified Dietician/Nutritionist (CDN) – New York
  • Experience with individuals with intellectual and developmental disabilities (IDD)
  • Excellent communication skills for health coaching and interdisciplinary collaboration

TITLE:  TREATMENT COORDINATOR

PROGRAM:  RESIDENTIAL

SUMMARY OF POSITION: The Treatment Coordinator is responsible for coordinating, monitoring, and ensuring the implementation of individualized treatment plans for individuals residing in OPWDD-certified residential homes. This role ensures that services are person-centered, compliant with OPWDD regulations, and aligned with each individual’s goals, needs, and safeguards. The Treatment Coordinator works collaboratively with interdisciplinary team (IDT) members, residential staff, families, Care Coordination Organizations (CCOs), and external providers.

This position carries responsibilities equivalent to those of a Residential Supervisor, including oversight of treatment plan implementation, staff guidance, and ensuring program compliance with OPWDD regulations.

Essential Duties and Responsibilities

Treatment Planning & Compliance

  • Coordinate the development, implementation, and monitoring of individualized treatment plans in accordance with OPWDD regulations and agency policies
  • Ensure treatment plans are person-centered, outcomes-focused, and reflective of the individual’s strengths, preferences, and needs
  • Monitor plan effectiveness and initiate updates or revisions as required
  • Ensure timely completion of required documentation, including Life Plans, addendums, assessments, and progress reviews

Interdisciplinary Team Coordination

  • Schedule, prepare for, and facilitate IDT meetings, including Life Plan meetings and periodic reviews
  • Collaborate with clinical staff, residential management, Direct Support Professionals (DSPs), nurses, behavior specialists, and external providers
  • Communicate treatment plan expectations clearly to residential staff and provide guidance on implementation

Documentation & Record Review

  • Review treatment plans, progress notes, and related documentation for accuracy, completeness, and regulatory compliance
  • Track due dates and ensure all required documentation is completed within OPWDD timelines
  • Maintain organized, audit-ready records in both electronic and paper formats

Staff Support & Training

  • Provide training and technical assistance to residential staff on treatment plan goals, strategies, and documentation requirements
  • Support supervisors in monitoring staff implementation of treatment plans
  • Identify areas where additional training or support is needed and communicate concerns to management

Quality Assurance & Oversight

  • Conduct periodic reviews of treatment plan implementation in residential settings
  • Participate in internal audits, quality improvement activities, and OPWDD surveys
  • Address deficiencies and assist in developing corrective action plans as needed

Coordination with External Partners

  • Liaise with Care Coordination Organizations (CCOs), clinicians, day programs, and other service providers
  • Support communication with families and advocates regarding treatment planning and progress

Supervisory and Program Oversight

Provide leadership and support to residential staff in implementing treatment plans and maintaining program standards. Collaborate with Supervisors and Area Coordinators to ensure continuity of care and operational compliance across multiple residences.

Qualifications 

Education

  • Bachelor’s degree in Human Services, Social Work, Psychology, Special Education, or a related field required

Experience

  • Minimum of two (2) years of experience working with individuals with intellectual and/or developmental disabilities
  • Experience in supervisory responsibilities within OPWDD residential programs required
  • Knowledge of person-centered planning and Life Plan requirements

Knowledge, Skills & Abilities

  • Strong knowledge of OPWDD regulations, treatment planning requirements, and documentation standards
  • Excellent organizational, time management, and attention-to-detail skills
  • Ability to work collaboratively with multidisciplinary teams
  • Strong written and verbal communication skills
  • Proficiency with electronic health records and Microsoft Office
  • Ability to manage multiple homes, priorities, and deadlines

Work Requirements

  • Ability to travel between residential homes in Queens and attend meetings

SALARY:  $66,300 annually


TITLE:  HANDYMAN

PROGRAM:  MAINTENANCE                       

SUMMARY OF POSITION: Under the close supervision of the Facilities Manager, the Handyman/Mechanic performs all daily inspections, preventive maintenance work and assists with general cleaning as needed.

DUTIES:  Repairs and installs mechanical electrical plumbing

Performs all daily inspections and preventive maintenance work.  Assists with general cleaning as needed.  Assists with garbage removal and handling.  Removes all snow when necessary.  Performs other duties as assigned

MINIMUM REQUIREMENTS:  High School/GED Diploma and 3 years of experience in general building maintenance work required. Knowledge of electrical, plumbing and HVAC required.

TITLE:  COMMUNITY PRE-VOCATIONAL INSTRUCTOR

PROGRAM:  PRE-VOCATIONAL

We are seeking a Full-Time Community Pre-Vocational Instructor for our Pre-vocational program.

Schedule: Mon – Fri 8:00 am to 3:30 pm

SUMMARY OF POSITION: The Pre-Vocational Instructor supports individuals with developmental disabilities in developing work related skills and preparing for future employment. The Instructor is responsible for transporting participants to and from volunteer sites, supervising their activities and ensuring their safety and well-being.

DUTIES:

  • Transport participants to and from volunteer sites
  • Supervise participants during volunteer activities and model positive work habits
  • Facilitate group programming after volunteer work to review and improve work skills and habits.
  • Address individual goals and track progress
  • Complete daily summaries of participant activities and achievements
  • Collaborate with team members and families to support participants’ growth and success.

POSITION SUMMARY: The Day Services Specialist facilitates day habilitation programming that promotes respect, safety, personal growth, and meaningful engagement for both individuals receiving services and staff members.

DUTIES:

  • Develop and maintain Staff Action Plans for assigned individuals
  • Conduct team meetings to review and discuss individual service plans
  • Complete monthly review notes summarizing individuals’ progress and responses to plans
  • Organize assigned groups and implement individualized day service plans
  • Serve as a liaison between families, care managers, and residential providers
  • Support a positive, person-centered environment that encourages independence and community involvement.

TITLE:  DAY HABILITATION ASSISTANT

PROGRAM:  BELLEROSE DAY PROGRAM

We are currently seeking a compassionate and motivated Day Habilitation Assistant to join our Day Program team in Bellerose, Queens.

SUMMARY OF POSITION: The Habilitation Assistant assists the Human Services Professional in all aspects of daily program participant programming.

DUTIES:

  • Motivate individuals to engage in program and community activities
