Service Coordination
QCP’s Service Coordination office helps people with developmental disabilities and their families access the necessary services and supports that are appropriate to their needs. The service coordinator is an advocate and facilitator, working with the individual and his or her family.
The focus of this service is to help each person achieve his or her unique goals and desires in terms of finding a home, a day program or a job, a circle of friends, and enjoyable leisure activities, or obtaining benefits to which they are entitled. It can also entail referral to competent medical doctor, dentist or other clinical personnel who understand the needs of people with developmental disabilities.
The service coordinators become partners with the people QCP serves, ensuring that the individuals are well informed and provided with appropriate choices and opportunities.
