SUMMARY OF POSITION:

Responsible for writing day habilitation plans and conducting team meetings.  Organizes consumer groups and implements day service plan for each consumer.  Acts as liaison with families, service coordinators and residential providers.

SUMMARY OF POSITION:

Responsible for conducting habilitation service training in the community with adults with developmental disabilities.  Duties include the completion of service documentation, writing narrative notes and planning group activities.

 

MINIMUM REQUIREMENTS:

High School Diploma/GED required.  Experience working with adults with developmental disabilities and a valid NYS driver’s license preferred.

Provides comprehensive care to adults with developmental disabilities in their homes.  Works on ADL goals, assists with personal hygiene needs and performs housekeeping tasks, escorts clients on recreational activities.

SUMMARY OF POSITION:

 

Responsible for overseeing staff in providing advocacy, linkage and referral for individuals with developmental disabilities and their families.   Responsible for overseeing staff assessing the needs of the person with developmental disability and developing a service plan that meets their valued outcome.

TITLE:  ACTIVITIES COORDINATOR

 

PROGRAM:  DAY SERVICES (164TH)

 

SUMMARY OF POSITION:

Screen and select Volunteer, Senior Center and Recreational sites in the community and serve as agency liaison to the sites.  Work cooperatively with supervisors and staff to make sure that community trips occur as planned.  Develop innovative service approaches and activities in the program area as well as large group theatrical productions, art shows and monthly or seasonal events.  Assist with the scheduling, organizing and daily upkeep of various program areas, e.g., the Greenhouse, Kitchen and Corner Store.

 

MINIMUM REQUIREMENTS:

BA/BS degree required.  Experience working with adults with developmental disabilities.

TITLE:  JOB COACH

 

PROGRAM:  SUPPORTED EMPLOYMENT

 

SUMMARY OF POSITION:  Provide on-site training for people with developmental disabilities by providing hands-on instruction and training in appropriate work habits and behaviors; perform job tasks for enrollees as necessary; work as a member of an interdisciplinary team, and function as a liaison between work site and agency.

 

MINIMUM REQUIREMENTS:  BA/BS and related experience required.  Clean NYS Driver’s license and own car required.

SUMMARY OF POSITION:

Ensures the implementation of Individuals’ program plan and integrates various clinical services.  Supervises staff, conducts annual and semi-annual conferences, writes comprehensive and detailed reports summarizing outcomes of program plans and ensures that program policies and procedures are carried out.

 

MINIMUM REQUIREMENTS:

Bachelor’s Degree in the human services field along with 1 year experience working with the developmentally disabled population is required.  In addition, prior supervisory experience and a valid New York State driver’s license for at least three (3) years is required.  Successful candidate should have comprehensive writing skills and excellent interpersonal skills.

The Human Services Professional will be responsible for assisting in the overall daily operations of the day program for adult individuals with special needs; participate in screenings, perform annual assessments & establish appropriate goals for program individuals.  Prepares and presents progress reports, as well as provides direct hands-on classroom services.  The successful candidate should have excellent writing skills.

Bachelor’s Degree in human services field and minimum one year experience in working with persons with developmental disabilities. Valid NYS Driver’s License for three (3) years is strongly preferred.

TITLE: RESIDENTIAL SUPERVISOR/QIDP    

           

PROGRAM: RESIDENTIAL         

 

SUMMARY OF POSITION: Responsible for the day to day operations of a residence for 10 individuals with developmental disabilities in accordance with OPWDD regulations and QCP policies.  In addition, directly provides supervision to all staff.  Ensures that all individuals are provided with an appropriate and individualized active treatment plan, which is monitored and revised accordingly.  Coordinates the IDT process and works closely with the Day & Vocational programs to provide a cohesive treatment plan, as well as ensures that family members are included in the IDT process.

 

MINIMUM REQUIREMENTS: Bachelor’s Degree & 1 year experience working with programs for developmentally disabled adults. Previous supervisory experience in an OPWDD setting is required, along with good writing and interpersonal skills.

PROGRAM: MAINTENANCE

 

SUMMARY OF POSITION:

Maintains general cleanliness of Agency’s buildings and grounds.  This includes repairing equipment, servicing, cleaning and sanitizing restroom, removing trash daily, painting areas as necessary, and cleaning and waxing floors.

 

MINIMUM REQUIREMENTS: High School/GED diploma and one year experience in building maintenance.
