TITLE:  DAY SERVICES ASSISTANT

PROGRAM:  DAY SERVICES (164th Street)

 

SUMMARY OF POSITION:

Conduct habilitation service training in the community with adults with developmental disabilities.  Duties include the completion of service documentation, writing narrative notes and planning group activities.

 

MINIMUM REQUIREMENTS:

High School Diploma/GED required.  Experience working with adults with developmental disabilities and a valid NYS driver’s license preferred.

Provide comprehensive care to adults with developmental disabilities in their homes.  Work on ADL goals, assist with personal hygiene needs and perform housekeeping tasks, escort clients on recreational activities.

 

MINIMUM REQUIREMENTS: High School Diploma or equivalent and valid New York State driver’s license required.  Prior work experience in a human service field and experience working with individuals with developmental disabilities preferred.

TITLE:  PRE-VOCATIONAL TRAINER

PROGRAM:  PRE-VOCATIONAL

 

SUMMARY OF POSITION:  Assign work and train clients on the required tasks for satisfactory completion of jobs.  Maintain daily records of attendance and productivity and provide written and verbal reports on client’s progress and performance.  Assume responsibility for quality control.

TITLE:  PRE-VOCATIONAL SUPERVISOR

PROGRAM:  PRE-VOCATIONAL

 

SUMMARY OF POSITION:

Provides supervision of the Pre-Vocational Instructors in the Pre-Vocational program. Oversees the development and implementation of vocational instruction including curriculum development, ordering supplies and materials, providing training to staff. Develops and implements service documents with consumers and their families. Plans, conducts and implements pre-vocational plans with consumers and their advocates.

Bachelor’s degree and 2 years of experience working in programs for developmentally disabled adults required

TITLE:  VAN SAFETY TRAINER

PROGRAM:  DAY PROGRAMS

 

SUMMARY OF POSITION:

 

Check vans for safety, train staff in: vehicle safety protocol, vehicle maintenance, inspections and outside repairs.

 

MINIMUM REQUIREMENTS:

Clean Driver’s license for five (5) years and basic knowledge of automobiles required.

TITLE:  HUMAN SERVICES PROFESSIONAL

PROGRAM:  BELLEROSE DAY PROGRAM

 

SUMMARY OF POSITION:

The Human Services Professional is responsible for assisting in the overall daily operations of the day program for adult individuals with special needs; participating in screenings, performing annual assessments & establishing appropriate goals for program individuals.  He/she prepares and presents progress reports, as well as provides direct hands-on classroom services.  The successful candidate should have excellent writing skills.

 

MINIMUM REQUIREMENTS:

Bachelor’s Degree in human services field and minimum of one year experience working with persons with developmental disabilities required. Valid NYS Driver’s License for three (3) years is strongly preferred.

TITLE:  COOK/HOUSEKEEPER

PROGRAM:  RESIDENTIAL

 

SUMMARY OF POSITION:

Duties entail preparing meals, purchasing food and household supplies, food inventory and performing housekeeping tasks such as mopping and laundry.

TITLE:  ASSISTANT RESIDENTIAL SUPERVISOR/QIDP

PROGRAM:  RESIDENTIAL

 

SUMMARY OF POSITION:

Coordinate, review and provide administrative supervision, encompassing staff, client and physical plant issues, to the ICFs, IRAs and other residential programs as designated by the Director.

 

MINIMUM REQUIREMENTS:   

Bachelor’s Degree and 1 year experience working in programs for Developmentally Disabled adults.  Previous supervisory experience in an OPWDD setting is required.

TITLE:  TEMPORARY QIDP

PROGRAM:  RESIDENTIAL

 

SUMMARY OF POSITION:

This temporary position will entail working approximately 12 hours per month in the Residential Program ensuring all individuals are provided with an appropriate person centered habilitation and protective oversight plan.  The QIDP is responsible to monitor plan implementation, oversee staff documentation and provide plan revisions and staff training as needed.  The QIDP will coordinate the IDT/ ISP process working closely with day/vocational and MSC programs ensuring family members or advocates are included in the decision making process as desired by the individual.

 

MINIMUM REQUIREMENTS:

Prior QIDP experience is required for this position (BA Degree in related field and 1 year of related experience in working with individuals with special needs).   Must have excellent writing skills, be a self-starter and be able to prioritize assignments.

TITLE:  PORTER

PROGRAM: MAINTENANCE

 

SUMMARY OF POSITION:

Maintain general cleanliness of Agency’s buildings and grounds.  This includes repairing equipment, servicing, cleaning and sanitizing restroom, removing trash daily, painting areas as necessary, and cleaning and waxing floors.

 

MINIMUM REQUIREMENTS: High School/GED diploma and one year experience in building maintenance.

CHILDREN’S CENTER OPENINGS

 

 

TITLE:  ADAPTIVE EQUIPMENT CARPENTER (PART-TIME)

PROGRAM:  CHILDREN’S CENTER                                               

 

SUMMARY OF POSITION:

The Adaptive Equipment Carpenter will construct adaptive equipment (and/or make necessary adjustments) as designed by therapists to meet the physical needs of the clients; maintain a high standard of safety regarding equipment produced and/or needed; maintain consistent on-site communication with staff involved in the creation of adaptive equipment; visit classrooms to make any repairs of equipment that classroom staff has noted; assist the Maintenance Department with various agency projects.

 

MINIMUM REQUIREMENTS:

High School Diploma is required.  Familiarity with the limitations and special physical needs of QCP’s clients preferred.
