TITLE:  DAY SERVICES ASSISTANT

PROGRAM:  DAY SERVICES (164th Street)

 

SUMMARY OF POSITION:

Conduct habilitation service training in the community with adults with developmental disabilities.  Duties include the completion of service documentation, writing narrative notes and planning group activities.

 

MINIMUM REQUIREMENTS:

High School Diploma/GED required.  Experience working with adults with developmental disabilities and a valid NYS driver’s license preferred.

SUMMARY OF POSITION:

Provide comprehensive care to adults with developmental disabilities in their homes.  Work on ADL goals, assist with personal hygiene needs and perform housekeeping tasks, escort clients on recreational activities.

 

MINIMUM REQUIREMENTS: High School Diploma or equivalent and valid New York State driver’s license required.  Prior work experience in a human service field and experience working with individuals with developmental disabilities preferred.

TITLE:  TRANSPORTATION WORKER

PROGRAM:  ADULT DAY PROGRAM

 

SUMMARY OF POSITION:

Transport passengers to and from designated destinations. As required, assist in entering and exiting vehicle in a safe manner and ensure that all passengers are appropriately secured and wheelchairs securely locked in position. Ensure operation of all safety and mechanical components and acceptable cosmetic condition of vehicle. Perform regular inspections and complete and submit Weekly Maintenance Report.

MINIMUM REQUIREMENTS:
Valid Driver’s License and clean driving record required.

TITLE:  PRE-VOCATIONAL SUPERVISOR

PROGRAM:  PRE-VOCATIONAL

 

SUMMARY OF POSITION:

Provides supervision of the Pre-Vocational Instructors in the Pre-Vocational program. Oversees the development and implementation of vocational instruction including curriculum development, ordering supplies and materials, providing training to staff. Develops and implements service documents with consumers and their families. Plans, conducts and implements pre-vocational plans with consumers and their advocates.

MINIMUM REQUIREMENTS:

Bachelor’s Degree and 2 years of experience working in programs for developmentally disabled adults required.

RESIDENTIAL PROGRAM OPENINGS

 

TITLE:  DIRECT SUPPORT ASSISTANT

PROGRAM:  RESIDENTIAL

 

SUMMARY OF POSITION:

TITLE:  ASSISTANT DIRECTOR OF RESIDENTIAL SERVICES

PROGRAM: RESIDENTIAL

           

SUMMARY OF POSITION:

The Assistant Director of Residential Services will report to the Director of Residential Services.  This person will be responsible for managing the day to day operations of several Residential Programs in accordance with OPWDD regulations and QCP policies.  In addition, ensures that facility and services provided meet the physical, social and developmental needs of all individuals in QCP’s care. 

 

MINIMUM REQUIREMENTS: Bachelor’s Degree in human services and 5 years’ experience managing residential programs for individuals with special needs.  The successful candidate is required to have extensive knowledge of OPWDD regulations and experience with audits & investigations. Strong leadership, interpersonal and communication skills, as well as experience in training all levels of staff.

TITLE:  SUPERVISOR

PROGRAM:  BELLEROSE DAY SERVICES

 

SUMMARY OF POSITION:

Responsible for the daily operation of assigned DH group rooms.  Supervises work performance of all QIDP staff with assigned DH group rooms.

 

MINIMUM REQUIREMENTS:

Bachelor’s Degree in Human Services Field and at least three (3) years of supervisory experience working in the OPWDD setting is required.

TITLE:  PORTER

PROGRAM: MAINTENANCE

 

SUMMARY OF POSITION:

Maintain general cleanliness of Agency’s buildings and grounds.  This includes repairing equipment, servicing, cleaning and sanitizing restroom, removing trash daily, painting are

as necessary, and cleaning and waxing floors.

 

MINIMUM REQUIREMENTS: High School/GED diploma and one (1) year experience in building maintenance.

High School diploma, Strong English skills required.

TITLE:  ASSISTANT TO DIRECTOR

PROGRAM:  CHILDREN’S CENTER     

           

SUMMARY OF POSITION:

The Assistant to Director of Children Services assists the administrative staff and supervisors with various program responsibilities and special assignments.  Interfaces with families and visitors, in person and on the phone. Provides training and support to staff on computer programs and troubleshoots.  Assists with front office and clerical functions.

 

Schedule:  Monday through Friday, 8:30 am ‑ 4:30 pm

 

MINIMUM QUALIFICATIONS:

BA/BS degree and/or minimum five (5) years administrative experience.  Must be highly organized and have excellent oral/written communications and computer skills in Word, Excel, Access and specialized software applications.
