SUMMARY OF POSITION:

TITLE:  JOB COACH

PROGRAM:  SUPPORTED EMPLOYMENT

 

SUMMARY OF POSITION:  Provide on-site training for people with developmental disabilities by providing hands-on instruction and training in appropriate work habits and behaviors; perform job tasks for enrollees as necessary; work as a member of an interdisciplinary team, and function as a liaison between work site and agency.

The Assistant Supervisor is responsible for assisting in the overall daily operations of the residence; supervises staff to enhance optimum services for residents.  The successful candidate should have excellent writing skills in order to write and implement goals.

 

MINIMUM REQUIREMENTS:

Bachelor’s Degree in human services required for the above position. Demonstrated leadership ability, valid NYS Driver’s License for three (3) years, minimum of one year experience working with individuals with developmental disabilities required.

TITLE:  RESIDENTIAL SUPERVISOR/QIDP   

PROGRAM:  RESIDENTIAL        

 

SUMMARY OF POSITION: Responsible for the day to day operations of a residence for 10 individuals with developmental disabilities in accordance with OPWDD regulations and QCP policies.  In addition, directly provides supervision to all staff.  Ensure that all individuals are provided with an appropriate and individualized active treatment plan, which is monitored and revised accordingly.  Coordinates the IDT process and works closely with the Day & Vocational programs to provide a cohesive treatment plan, as well as ensures that family members are included in the IDT process.

 

MINIMUM REQUIREMENTS: Bachelor’s Degree & 1 year experience working with programs for developmentally disabled adults. Previous supervisory experience in an OPWDD setting is required, along with good writing and interpersonal skills.

TITLE:  RN SUPERVISOR

PROGRAM:  RESIDENTIAL

 

SUMMARY OF POSITION:

Under the direct supervision of the Residential Director, the Nursing Supervisor will be responsible for ensuring that all residential individuals receive quality medical/nursing care, and that all needed and required medical services are in compliance according to government regulations and Agency policy. Supervises nurses, develops and monitors professional career goals for nursing staff. Other duties include overseeing coordination and supervision of medical, dental, and other health related care services, laboratory procedures, results and follow-up tests and consultation with Primary Care Physician to review all individual medical concerns. Develop and implement policies and procedures to ensure individual safety and well-being in accordance with OPWDD regulations and agency policy. Monitor all medical systems for compliance. Participate in the auditing process and guides and directs nursing participation as needed.

MINIMUM REQUIREMENTS:

BSN degree, Current NYS RN license, and minimum 2 years working in programs for persons with developmental disabilities required.

The Human Services Professional is responsible for assisting in the overall daily operations of the day program for adult individuals with special needs; participating in screenings, performing annual assessments & establishing appropriate goals for program individuals.  He/she prepares and presents progress reports, as well as provides direct hands-on classroom services.  The successful candidate should have excellent writing skills.

SUMMARY OF POSITION:

Maintain general cleanliness of Agency’s buildings and grounds.  This includes repairing equipment, servicing, cleaning and sanitizing restroom, removing trash daily, painting areas as necessary, and cleaning and waxing floors.

TITLE:  PEDIATRIC OCCUPATIONAL THERAPIST (Per Diem)

TITLE:  ADAPTIVE EQUIPMENT CARPENTER (PART-TIME)

PROGRAM:  CHILDREN’S CENTER                                               

 

SUMMARY OF POSITION:

The Adaptive Equipment Carpenter will construct adaptive equipment (and/or make necessary adjustments) as designed by therapists to meet the physical needs of the clients; maintain a high standard of safety regarding equipment produced and/or needed; maintain consistent on-site communication with staff involved in the creation of adaptive equipment; visit classrooms to make any repairs of equipment that classroom staff has noted; assist the Maintenance Department with various agency projects.

 

MINIMUM REQUIREMENTS:

High School Diploma is required.  Familiarity with the limitations and special physical needs of QCP’s clients preferred.

TITLE:  ASSISTANT DIRECTOR – IT

PROGRAM:  MIS

 

SUMMARY: Under the general direction of the Director of IT, manages all aspects of the Agency’s information technology network, structure, and security protocols.

Responsibilities encompass the management and monitoring of technologies that include but are not limited to; monitoring and implementing HIPPA security systems, integrating systems and programs to ensure current standards within the field are met; monitoring and securing the transmission of protected health information (PHI), identifying needs of users and designing reports, interfacing with software companies, providing ongoing training to staff in use of technology; updating technology policies and procedures, conducting administrative level audits to ensure compliance with regulations.

 

PRINCIPAL DUTIES AND RESPONSIBILITIES

  • Participates in the development of the IT strategies and systems
  • Implements and performs security, software maintenance, data management, recordkeeping, and storage protocols.
  • Identifies information system needs, and develops protocols through utilizing analytics and the formulation of data
  • Optimizes use of the entire system and network by performing needs analysis, procurement, support, installation, training and problem resolution.
  • Ensures proper implementation, configuration, monitoring and maintenance of security firewalls, applications and device servers and Internet, connections and encryption.
  • Maintains all connectivity to all third party business partners.
  • Supervises and monitors the Help Desk system.
  • Performs Risk Analysis and management
  • Develops, maintains and updates a library of up-to-date technical documentation.
  • Assists in the maintenance of customized in-house applications.
  • Trouble shoots hardware and software problems.

 

Education and/or Experience                                                  

Bachelor’s degree in technology or related field and a minimum of five years high level technical experience required. 

Must have documentable, detailed knowledge of   VMware, Windows Server 2008/12, Windows Exchange Server 2010/16, Microsoft SQL Server, Microsoft Windows 7/8/10, Microsoft Office Suite 2010/13, TrendMicro antivirus program, Google Apps, Symantec=s BackupExec software, WordPress, Java, Java script, ASP, HTML, FTP, and  PhotoShop.
