TITLE: PRE-VOCATIONAL SPECIALIST

PROGRAM: PRE-VOCATIONAL

SUMMARY OF POSITION: Develop and implement service documents with consumers and their families.  Maintain a caseload based on program needs.  Plan, conduct, and implement pre-vocational plans with consumers and their advocates. Conduct and complete documents required for VESID intakes at the Queens VESID office.

MINIMUM REQUIREMENTS:
BA in Rehabilitation Counseling or other related field, experience in working with persons with Developmental Disabilities preferred.

TITLE: DIRECT CARE WORKER - VARIOUS SHIFTS FT, PT, Per Diem

PROGRAM: RESIDENTIAL

SUMMARY OF POSITION: Provide comprehensive care to adults with developmental disabilities in their homes. Work on ADL goals, assist with personal hygiene needs, perform housekeeping tasks and escort clients on recreational activities.

MINIMUM REQUIREMENTS: HS Diploma or equivalent, valid New York State driver’s license required for one year. Prior work experience in a human service field with individuals with physical disabilities is preferred.

TITLE: NIGHT SUPERVISOR

PROGRAM: RESIDENTIAL

SUMMARY OF POSITION: Provide oversight of the overall care of the individuals residing on the Bellerose campus during the night shift hours.  The Night Supervisor will assist Residence Supervisors and Shift Supervisors to ensure the implementation of the plan of care in all areas of daily living in the early morning hours.

MINIMUM REQUIREMENTS: HS Diploma or equivalent, valid New York State driver’s license required for one year. Prior work experience in a human service field with individuals with physical disabilities is preferred.

TITLE:  MEDICAID SERVICE COORDINATOR

PROGRAM:  COORDINATION AND AT-HOME SERVICES

SUMMARY OF POSITION:  Provide advocacy, linkage and referral for individuals with developmental disabilities and their families. He/she is responsible for assessing the needs of the person with DD and developing a service plan that meets their valued outcomes. Duties include: Develop and review Individualized Service Plans (ISPs) for each consumer in assigned caseload, Conduct face to face interviews with the consumer on a monthly basis and a home visit at least every quarter, Maintain current MSC records per regulatory requirements, Follow up and monitor consumer needs and facilitate access to programs and services, Participate in supervisory sessions and training requirements of the program.

MINIMUM REQUIREMENTS: Bachelor's Degree in Human Service field; one year experience in working with persons with developmental disabilities required.

TITLE: QUALITY ASSURANCE SPECIALIST

PROGRAM: QUALITY ASSURANCE

SUMMARY OF POSITION: Assist in program audits and following up on training issues, assist in investigations, develop and execute training programs in compliance with State and Federal regulations, attend internal and external meetings, conferences, and trainings, and provide technical assistance to Agency programs as needed.

MINIMUM REQUIREMENTS: BA degree plus experience in working within OPWDD regulations in residential programs required.

TITLE: MSC RECORDS COORDINATOR

PROGRAM: MSC

SUMMARY OF POSITION: Duties include providing intake service - clinic assessment & report for admission to the MSC department; generating and tracking the timely notification, completion, and distribution of ISPs and other documents; preparing routine reports and special tabulations on client related information; reviewing client records for regulatory compliance, recognizing and correcting inconsistencies, reviewing case notes and treatment plans to ensure compliance with documentation requirements, and responding to internal and external clinic audits.

MINIMUM REQUIREMENTS: BA degree, MSC training, minimum 2 years MSC experience and administrative skills.
