TITLE: ASSISTANT DIRECTOR - MEDICAID SERVICE COORDINATION

PROGRAM: MEDICAID SERVICE COORDINATION

SUMMARY OF POSITION: Managing and directing our Service Coordination Program, providing direction and ensuring compliance with all regulatory mandates governing Clinic operations, overseeing supervisors and staff in provision of services, ensuring the appropriate and timely completion of treatment notes and plans, clinical reports and evaluations, participation in internal and external committees as required.

MINIMUM QUALIFICATIONS: Master's Degree in Health Care Administration or in a health related field, minimum 3 years administrative and supervisory experience in the delivery of clinical services to individuals with developmental disabilities.

TITLE: DIRECT CARE WORKER - VARIOUS SHIFTS FT, PT, Per Diem

PROGRAM: RESIDENTIAL

SUMMARY OF POSITION: Provide comprehensive care to adults with developmental disabilities in their homes. Work on ADL goals, assist with personal hygiene needs, perform housekeeping tasks and escort clients on recreational activities.

MINIMUM REQUIREMENTS: HS Diploma or equivalent, valid New York State driver’s license required for one year. Prior work experience in a human service field with individuals with physical disabilities is preferred.

TITLE: COOK/HOUSEKEEPER FT

PROGRAM: RESIDENTIAL

SUMMARY OF POSITION: Duties entail preparing meals, purchasing food and household supplies, maintaining food inventory and performing housekeeping tasks such as mopping and laundry. 

HS Diploma or equivalent, along with a valid New York State driver’s license for one year is required.

TITLE: SHIFT SUPERVISOR FT

PROGRAM: RESIDENTIAL

SUMMARY OF POSITION: Assist in overseeing the day-to-day operations of a home for adults with developmental disabilities. Coordinate staff schedules, plan and implement recreational activities, maintain resident and house funds, and oversee the maintenance of facility.

MINIMUM REQUIREMENTS: Associate’s degree preferred plus minimum 2 years related experience. NYS Driver’s License required. Must be able to work evenings and weekends.

TITLE: JOB COACH

PROGRAM: SUPPORTED EMPLOYMENT

SUMMARY OF POSITION: Provide on-site skills training for people with developmental disabilities, perform job tasks for enrollees as necessary and function as a liaison between work site and agency.

MINIMUM QUALIFICATIONS: HS Diploma or equivalent (some college preferred) and related experience.  NYS  Driver’s license and own car required.

TITLE: QUALITY ASSURANCE SPECIALIST Full Time (will consider temporary per diem until position is filled on a Full Time basis)

PROGRAM: QUALITY ASSURANCE

SUMMARY OF POSITION: Conducting Compliance auditing and training, assisting in investigations, developing and executing training programs in compliance with State and Federal regulations, attending internal and external meetings, conferences, and trainings, and providing technical assistance to Agency programs as needed.

MINIMUM QUALIFICATIONS: BA degree plus three years experience in auditing and training in accordance with OMRDD regulations required.

TITLE: PHYSICAL THERAPY /PTA/ FULL OR PART TIME

PROGRAM: RESIDENTIAL/DAY PROGRAM

SUMMARY OF POSITION: Maintain consumer mobility and improve quality of life through gross and fine motor skills, range of motion, strength and muscle tone exercises and activities of daily living. Assess consumers for P.T. services; develop ROM and a variety of individual and group exercise programs. Participate in screening of new consumers. Write semi and annual reviews, be a member of the interdisciplinary team, and attend clinical meetings.

MINIMUM QUALIFICATIONS: Degree in Physical Therapy, State License and Registration.

TITLE: RESIDENTIAL HABILITATION WORKER - P/T  Per Diem

PROGRAM: CLINIC

SUMMARY OF POSITION: Work one to one with individual with a developmental disabilities in their homes.  Support these individuals in skill building; provide relief to primary care takers.

MINIMUM REQUIREMENTS: HS Diploma or equivalent, valid New York State driver’s license required. Prior work experience in a human service field with individuals with physical disabilities is preferred.

TITLE: DEVELOPMENT ASSISTANT

PROGRAM: FUNDRAISING/ DEVELOPMENT

SUMMARY OF POSITION: Duties include arranging special events, carrying out telephone solicitations, doing financial reconciliations, taking minutes at meetings.

MINIMUM REQUIREMENTS: AS degree and 3 years related experience. Excellent verbal and written communication skills required. Must also have excellent computer skills including proficiency in Access, Excel, Photoshop, Online Fund-raising, and Database experience.
